Lentara UnitingCare is committed to maintaining the privacy of all individuals according to our statutory requirements regarding how we collect, hold, use, disclose, correct, transfer and dispose of information. Collection and purpose

Information is usually collected from you, or a third party with your consent, so that we can provide you with services that you consider to be most appropriate.

We use your information to provide quality care and suitable services. Some of these services are provided by Lentara UnitingCare, some services are provided by other community service providers or government offices.

We understand that your information is sensitive and we will not use if for a secondary purpose unless that purpose is directly related to the primary purpose of collecting the information. When we need to speak with other people about your circumstances, we will ask your consent before speaking with others (third parties), unless we are otherwise required by law. You have a right not to give consent however this may limit our services to you. If this is the case we will let you know.

Right to access and correction

We will keep information about you as accurate, complete, relevant and up to date as possible. You have a right to access and correct information if it’s established as incorrect, misleading, inaccurate, incomplete or out of date.


We will dispose of your information, as soon as practicable when you stop receiving a service from Lentara UnitingCare and when it is no longer required to be kept by law.

We make all reasonable attempts to keep your information secure from unauthorised access, use, loss, alteration and interference.
